Metropolitan Health Providers Billings Service (MHPBS) provides billing services for Lori and can be reached at (503) 249-0181. Lori no longer takes private insurance and works with CareOregon, accepting HealthShare only. As a provider, if you do receive a bill from our billing service, please call Metropolitan Health Providers Billings Service for further information.

IMPORTANT INFORMATION

MEDICATION: Medication refills can take up to 5 days. Stimulant medication prescriptions for ADHD must be provided each month and cannot be written for refill. Please plan accordingly. Medication refill requests are honored for Current Patients only. Patients who are considered a "Current Patient" are those who are seen at least every six months. Cycle medications are refilled electronically only. Controlled medications are not on cycle so the provider must call at least 5 days prior to requiring the medication and the pharmacy should ask me for a refill request electronically.

Home visits are typically every three months unless otherwise necessary. Specific times are scheduled but there are many variables that may require that time to be earlier or later. Please be prepared all day for the appointment and this includes keeping your resident home for the day. All paperwork must be completed during the appointment which includes the physician order forms or consultation forms and most importantly the balancing test that are required for each psychotropic medication once per year.
